Special report: MEG and MSI for the purpose of presurgical localization of epileptic lesions--a challenge for technology evaluation.

چکیده
Authors' objectives The objective of this Special Report is to present the particular challenges that this diagnostic technology presents in terms of evaluating the evidence to determine its potential benefit. Without a thorough presentation of the manner in which MEG is used in the evaluation of patients, it is difficult to determine whether the technology is beneficial. Various problems such as ascertainment biases, patient dropouts, and imperfect reference standards crop up in the published studies evaluating MEG, making it more difficult to assess them. To some extent, these difficulties may be inherent in the clinical setting in which MEG is used.

Clinical relevance of source location in frontal lobe epilepsy and prediction of postoperative long-term outcome
PURPOSE To evaluate the value of magnetoencephalography (MEG) source localization in localization of epileptic activities and predicting surgical outcome in frontal lobe epilepsies (FLE). METHODS Forty-six patients with presurgical MEG evaluation and intractable FLE surgery (28 male patients) were analyzed retrospectively with a mean follow-up of 5 years. Dipole analysis was performed for MEG...
متن کاملMagnetic brain source imaging of focal epileptic activity: a synopsis of 455 cases.
Epilepsy surgery is based upon the minute assessment of brain tissue generating epileptic activity. A number of diagnostic methods are employed in the process of presurgical evaluation, supplying information on various morphological and functional aspects, ultimately integrated into the general result fundamental to the final treatment decision. Magnetic source imaging (MSI), combining structur...
متن کاملPeriventricular nodular heterotopia: A challenge for epilepsy surgery
Pharmacoresistant focal epilepsies due to periventricular nodular heterotopia are a diagnostic and therapeutic challenge because of the need of invasive presurgical diagnostics and the selection of an optimal surgical approach. Invasive investigations in previous studies showed that focal epileptic activity can be correlated predominantly either with one of the nodular heterotopia or with neoco...
متن کاملThe effect of incorporating the quantitative analysis besides visual assessments of 18F-FDG brain PET images for the localization of epileptogenic zones
Introduction: FDG Brain PET is a valuable paraclinical tool for presurgical assessments of patients suffering from refractory epilepsy. By the widespread accessibility of PET, recognizing the functional lesions has become a current practical method especially in preoperative evaluations of the partial epilepsy disorders. The aim of our study was to assess the impact of quantit...
متن کاملEEG Source Imaging in Partial Epilepsy in Comparison with Presurgical Evaluation and Magnetoencephalography
BACKGROUND AND PURPOSE The aim of this study was to determine the usefulness of three-dimensional (3D) scalp EEG source imaging (ESI) in partial epilepsy in comparison with the results of presurgical evaluation, magnetoencephalography (MEG), and electrocorticography (ECoG). METHODS The epilepsy syndrome of 27 partial epilepsy patients was determined by presurgical evaluations. EEG recordings ...
